Our Telepsychiatry service is ideal for people with busy schedules and parents who both want to be present for their child’s appointment, but coordinating schedules is challenging. Preparing For Your First Appointment
To make an appointment with Doctor Sam, please call (561) 510-9150. Doctor Sam will personally return your phone call and will provide you with a brief phone consultation free of charge. After your conversation with Doctor Sam, you will be scheduled with an appointment date and time. Changes to appointments must be made no less than 72 hours before your scheduled appointment time. Within 1 day of scheduling your appointment, you will receive an email to review and electronically sign Doctor Sam’s new patient forms. These forms must be completed prior to your appointment. You will also receive a personal invitation to your patient portal, where you will complete a diagnostic questionnaire. The diagnostic questionnaire should be completed at least 24-hours before your appointment. These documents will be reviewed directly by Doctor Sam who will be aware of the reason for your visit and your history. Your upcoming appointment will be tailored accordingly to your individual needs and requests.
New Patient Forms
Upon scheduling your appointment, you will receive a personalized email with access to all new patient forms. They can be conveniently completed electronically prior to your initial evaluation.
Patient Portal
Every patient, free of charge, will have access to a personalized portal where you can access appointments, forms and questionnaires administered by Doctor Sam. Prior to your initial evaluation, there will be a diagnostic questionnaire available in your portal. This questionnaire should be completed at least 24-hours prior to your appointment.

DOES DOCTOR SAM DO MEDICATION MANAGEMENT, THERAPY OR BOTH?
Doctor Sam provides both medication management and therapy. Doctor Sam has devoted years to taking therapy courses that complement the psychiatric services offered in the practice. Doctor Sam’s extensive training in both therapy and medication management is rather unique to the field of psychiatry. Some patients may only need medication management. Others may need therapy as well. With Doctor Sam, together you will determine the best treatment plan for you or your child.
How long is a typical session with Doctor Sam?
Sessions vary in length depending on the services provided. Sessions with therapy typically last longer than sessions that are exclusively for medication management, unless more time is needed. If you have a child who is being seen by Doctor Sam, Doctor Sam will ask to spend time with both you and your child. Sessions can also be scheduled back to back upon request. WHAT IS THE WAIT TIME FOR DOCTOR SAM?
While there may be a wait to see Doctor Sam, a member of Doctor Sam will try and provide you with an appointment as soon as possible, particularly if you are flexible with scheduling. If there is an urgent matter, Doctor Sam will try and accommodate. Doctor Sam treats each patient as if they are family and will provide you with the best care and individualized treatment available.
Do you accept insurance?
While Doctor Sam does not accept insurance, we are happy to provide you with a superbill that can be submitted to your insurance company if you have out-of-network benefits.
CAN I USE MY INSURANCE TO HELP COVER THE COSTS OF TREATMENT?
Upon request, we will provide you with a form that you can submit to your insurance company for possible reimbursement should you choose to use your insurance. Insurance companies vary as to how much they will reimburse and insurance companies may not be able to cover the full cost of treatment. This is because Doctor Sam devotes a unique amount of time, dedication and individualized care to each of her patients. This allows her to provide a higher level of care that some in network providers may not be able to give. Doctor Sam is dedicated to your treatment.
CAN DOCTOR SAM ELECTRONICALLY PRESCRIBE MEDICATIONS FOR ME?
Yes! Doctor Sam has full e-prescribing capabilities. You don’t need to worry about having a physical prescription. Prescriptions can be sent directly to the pharmacy of your choice and be picked up at your earliest convenience.
HOW LONG DOES IT TAKE FOR MEDICATIONS TO START WORKING?
